Arrowleaf in Vienna, IL is seeking a Counselor/Therapist to join our behavioral health team. The role involves counseling, coordinating care, and providing crisis interventions as part of a community-based program.

Full-time position with a 40-hour work week and periodic weekend crisis rotation. Immediate supervisor is the Programs Coordinator, and duties include case management and collaboration with a multidisciplinary team.

Responsibilities

• Intervenes and evaluates consumers; establishes preliminary diagnosis.
• Develops and monitors treatment plans with consumers; defines goals, outcomes, and timeframes.
• Monitors consumer behavior and prepares reports for medical necessity in evaluations.
• Intervenes therapeutically to improve attitudes, beliefs and behaviors.
• Provides crisis intervention to stabilize conditions; may refer to other services.
• Maintains networks with internal and external human service providers.
• Carries a caseload of clients enrolled in Community Support Team services.
• Completes required monthly direct service hours.
• Delivers case management and therapeutic interventions in community settings.
• Facilitates recreation, socialization, and cultural activities for clients.
• Teaches life-management skills and assists with independent living activities.
• Provides and arranges transportation services where needed.
• Participates in supervision, team meetings, and clinical staffing.
• Coordinates with interdisciplinary teams to meet clients’ needs.

Job Description

Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services.

Hours of Work

Full-time, 40 hours/week. Participates in afterhours weekend call crisis rotation in the 830 Residential Houses at least once a month.

• Develops and monitors treatment plan collaboratively with consumers by defining goals, specific outcomes and time-frames for treatment.
• Monitors consumer's behavior and makes detailed reports for medical necessity in regular comprehensive evaluations.
• Intervenes therapeutically to change attitudes, beliefs and behaviors of consumers to improve individual and family level of functioning.
• Provides crisis intervention to stabilize consumer's condition by utilizing short-term crisis intervention techniques and/or referral to other support services.
• Maintains networking and referral contacts with other human service providers bothinternally and externally.
• Carries a caseload of clients enrolled in Community Support Team services and as assigned by direct supervisor.
• Completes required monthly direct service hours to consumers as defined on Direct Service Requirement sheet.
• Facilitates comprehensive consumer care by providing case management services and therapeutic intervention in community-based settings.
• Facilitates recreation/socialization and cultural activities for consumers which provide enjoyable outings that prepare and encourage them to engage in activities independent of the agency.
• Effectively teaches life-management skills to consumers and assist with the development of independent living activities, such as grocery shopping, attending appointments, social skills training, money management, and coping skills.
• Provides and arranges transportation services to consumers which they translate to their natural environment.
• Demonstrates active involvement in supervision, team meetings, and clinical staffing.
• Collaborate withinternaland external members of the interdisciplinary team to coordinate services and ensure the clients needs are being met.
