Qualifications

• Graduate of accredited school of nursing or education equivalency for licensing.

Responsibilities

• Obtains comprehensive patient data from patient, family and other providers to develop a holistic view of the patient’s needs or condition.
• Analyzes data to determine diagnoses or conditions relevant to care.
• Identifies optimal outcomes for the patient in collaboration with the patient, family and interprofessional team.
• Develops a plan prescribing strategies and alternatives to attain outcomes with clinical judgement.
• Implements the plan to promote and maintain a safe environment and coordinates with the team.
• Evaluates progress toward goals and outcomes.
• Maintains current knowledge and competences; contributes to nursing practice.
• Communicates effectively with the patient, family and interprofessional team.
• Applies evidence-based practice and integrates scientific knowledge in care for age-specific needs.
• Works in a constant state of alertness and safety.
• Performs other duties as assigned.
