Qualifications

Responsibilities

• Performs initial assessment of patient.
• Forms plan of care and nursing diagnosis based on assessment.
• Revises and updates plan of care through ongoing evaluation.
• Delivers nursing care safely and timely per policies.
• Documents care accurately and educates patient for discharge.
• Delegates duties to team members with accountability.
