Qualifications

Responsibilities

• Assess biophysical, psychosocial, environmental and discharge planning needs.
• Document nursing assessments and patient responses accurately.
• Develop and implement individualized care plans using nursing diagnoses.
• Coordinate with family, caregivers and other disciplines for care planning.
• Provide patient/family education and discharge planning.
• Deliver nursing interventions safely and effectively according to standards.
• Demonstrate leadership, mentorship, and delegation to staff as needed.
• Adhere to FH policies, regulatory standards, and clinical ladder requirements.
