Job summary

BrightPath seeks a Program Coordinator to lead a new short-term crisis stabilization home in the Twin Cities. You will work five direct care shifts weekly, in ratio, alongside the team you train.

You’ll launch routines, medication systems, and documentation correctly from day one. You’ll build the program’s intake, care planning, and transition processes, coaching staff in real-time and ensuring high-quality care for youth in crisis within a 90-day target.

Qualifications

• 50 hours of education/training related to human services and disabilities
• Experience supporting a vulnerable population in a healthcare oversight capacity
• Leadership experience and/or training in disability services
• Ability to collaborate with a team on healthcare needs, care plan management, and IDT coordination
• Proficiency with electronic documentation; Therap E-MAR and Google Workspace experience
• Must pass DHS background study and maintain status
• Current valid driver’s license and insurance as required by state law

Responsibilities

• Provide direct care and support based on each person's Support Plan and Addendum
• Support daily cares, ADLs, medical needs, and mental health supports
• Implement person-centered programming aligned with the Plan and Addendum
• Assist in developing goals, monitoring progress, and collecting program data
• Complete and distribute progress reports
• Participate in interdisciplinary team meetings and care planning
• Handle meal planning, grocery shopping, and household supplies, ensuring three meals daily
• Oversee site cleanliness and ensure shift cleaning tasks are completed
• Oversee healthcare coordination including medication orders and inventory
• Coordinate medication reconciliation at intake and prepare documentation for discharge
• Train new staff and coach the team on care plans and active treatment plans
• Maintain binders to licensing standards and coordinate scheduling for coverage
• Ensure compliance with licensing standards and document daily logs accurately
• Budget responsibilities: Divvy transactions and receipts
