Job summary

BrightPath is seeking a Program Coordinator to lead on-site care for adolescents with intellectual/developmental disabilities and challenging behaviors in Twin Cities homes. You’ll supervise staff, ensure care plans are followed, and manage medication documentation while maintaining safety and compliance.

The role is full-time, evenings 2:00 PM–10:00 PM, 4 days a week with weekend rotation, and pays $30 per hour plus benefits and career growth opportunities.

Qualifications

• Experience supporting a vulnerable population in healthcare or oversight.
• Leadership experience and/or training in disability or behavioral services.
• Ability to pass a DHS background study and maintain qualified status.
• Current valid driver’s license and insurance.
• Willingness to participate in on-call rotation.

Responsibilities

• Provide direct care based on each individual's plan and addendum.
• Be the go-to leader on shift; train new staff and explain care plans.
• Own medication management and coordination with RN.
• Document ISP data, daily logs, and quarterly progress reports.
• Participate in interdisciplinary team meetings and contribute to care planning.
• Ensure home compliance with 245D and licensing standards; manage meals and budget.
