Job summary

The West Michigan Hispanic Chamber of Commerce seeks a Case Manager – Specialized Care & Emergency Services to join our team in Kalamazoo. This full-time, day-shift role starts at $50,440 per year and offers opportunities to support individuals in behavioral health crises through collaborative, trauma-informed care.

Responsibilities include crisis assessment, connecting clients with community resources, and working with partners to determine the appropriate level of care.

Qualifications

• Bachelor's degree in social work required.
• Minimum of one year experience with adults with serious mental illness, substance abuse, and/or co-occurring disorders.
• Experience with youth with serious emotional disturbances and co-occurring disorders is a plus.
• A valid Michigan driver's license and licensure (LBSW or LLBSW) required.

Responsibilities

• Provide short-term crisis services as part of the ICS team.
• Deliver collaborative, strengths-based, trauma-informed care to individuals and families.
• Evaluate individuals and families and connect them to community resources and services.
• Coordinate with community providers to assess and determine the appropriate level of care.
