Job summary

Community Healing Centers in Kalamazoo, MI, is seeking a part-time Case Manager for the Children’s Program. You will design and implement service plans, coordinate supports, and monitor outcomes to help children and families access health, housing, education, and other essential services.

The role requires a minimum Bachelor’s Degree, LLBSW/LBSW for social work, and at least three years of experience with children and families.

Qualifications

• Minimum Bachelor’s Degree in Early Childhood Development, Education, Social Work or other related field.
• Bachelor level Social Worker must have an LLBSW or LBSW.
• Three years’ experience working with children and families.
• Ability to establish and maintain effective therapeutic relationships with clients and families.
• Reliable transportation and valid driver’s license.
• Flu and Covid-19 vaccines required; booster preferred.

Responsibilities

• Prepare case histories for use in diagnosis and treatment.
• Advocate for, link, coordinate and monitor children and family needs with appropriate agency involvement.
• Provide comprehensive case management services, including linking families to services and monitoring progress.
• Attend meetings with Kalamazoo partners and school staff to collaborate services.
• Participate in multi-disciplinary teams to assess and review needs of children and families.
• Provide transportation when medically necessary.
• Maintain accurate case documentation and timely reporting of services.
• Coordinate crisis intervention and hospitalization screening during business hours.
