Job summary

SUN Behavioral Health in Kentucky seeks a licensed mental health professional to perform comprehensive psychosocial assessments, coordinate admissions, and connect patients with outpatient and inpatient services. You will work within a multidisciplinary team, conduct individual and group therapy, participate in treatment planning, and maintain documentation to support patient safety and continuity of care.

The role requires current licensure (LPC/LMFT/LCSW or equivalent), CPR/de-escalation

Qualifications

• Current unencumbered license in state of employment as LPC, LMFT, LSW, LISW, LISW-S, LPCC, LPCC-S, LMSW, or LCSW, or state equivalent license.
• CPR and hospital-selected de-escalation technique certification.
• Minimum: two (2) years of supervised clinical experience.

Responsibilities

• Completes psychosocial evaluations on assigned patients/clients.
• Interviews patients/clients, family members and others to identify precipitating events and disruption.
• Assesses psychosocial stressors to minimize crisis impact.
• Facilitates emergency detention process and documents appropriately.
• Provides age-appropriate patient care including pediatric and geriatric populations.
• Conducts therapy sessions with individuals and groups.
• Maintains knowledge of psychopathology and treatment approaches.
• Writes assessments and diagnoses for medical records.
• Participates in weekly treatment planning and discharge planning.
• Engages in discharge planning with patient, physician, and staff.
• Collaborates with community resources for evaluation and referrals.
• Documents all patient interactions for records and third-party payers.
• Performs triage to refer patients to Assessment and Referral Department.
• Maintains safety and quality in patient care.
• Acts as an advocate in the community.
• Promotes safe workplace interactions.
