Responsibilities

• Provide assessment and crisis intervention in various settings including crisis care center, home, and community via mobile services.
• Develop and implement individualized service plans for crisis stabilization.
• Coordinate services across multiple systems to ensure least restrictive interventions.
• Complete GPRA/NOMs and other required assessments.

• Attend team meetings and contribute to program development.
