Job summary

Connections Health Solutions is seeking a Registered Nurse to assess consumers in crisis, document health information, and participate in multidisciplinary care. The role emphasizes crisis intervention, triage, and medication administration within a physician-led, data-driven model.

The RN will educate patients and families about behavioral health diagnoses and treatments, support de-escalation, and follow Seclusion/Restraint guidelines. Full-time with comprehensive benefits offered.

Qualifications

• Graduate of an accredited Nursing program with an active RN license.
• Unrestricted AZ RN license or multi-state privilege.
• Meet the company vaccination policy and start requirements.
• Experience with crisis care and de-escalation is preferred.

Responsibilities

• Conduct detailed nursing assessments including triage and health assessments.
• Plan individualized treatment as part of the multidisciplinary team.
• Educate patients and families about diagnosis, medications, and treatments.
• De-escalate potentially dangerous situations using verbal interventions.
• Adhere to Seclusion/Restraint standards and complete required documentation.
