Qualifications

• Master’s degree in social work or related field from an accredited institution.
• Current state licensure as LCSW, LMFT, CMHC/LCPC/LP or Psychologist.
• Excellent written and verbal communication skills.
• Computer skills including email and word processing.
• Experience with patients, families, and treatment teams.

Responsibilities

• Diagnoses and treats mental, emotional and behavioral disorders and updates care plans.
• Refer patients to community services, treatment programs, or other providers.
• Develop discharge plans with a multi-disciplinary team to ensure continuity of care.
• Conduct psychosocial assessments and provide crisis interventions as needed.
• Educate and coordinate with patient care teams and external agencies.
