Job summary

Intermountain Health is seeking a Clinical Mental Health Professional to perform psychosocial assessments, develop comprehensive care plans, and coordinate crisis referrals within a 24/7 behavioral health setting in Murray, Utah.

You will collaborate with a multidisciplinary team to support patients and families, arrange discharge planning, and ensure smooth transitions to community resources. A Master’s degree and active state licensure are required.

Qualifications

• Master’s degree in social work or related field from an accredited institution.
• Current state licensure as LCSW, LMFT, CMHC/LPC/LCPC, LMFT as required by state.
• Excellent written and verbal communication skills.
• Computer skills including email and word processing.

Responsibilities

• Diagnoses and treats mental, emotional, and behavioral disorders; modify care plans as patient status changes.
• Refer patients to community services, treatment programs, support groups, psychiatrists, or other mental health providers.
• Develop discharge plans with the multidisciplinary team to ensure continuity of care.
• Organize and prioritize daily work by assessing patient needs.
• Perform psychosocial assessments and provide crisis, bereavement, and other behavioral health interventions as needed.
• Educate and coordinate with multiple patient care teams and outside agencies to ensure quality care and timely discharge.
• Provide staff support and debriefing following adverse events, as needed.
