Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field is required.
• Minimum of 2 years of case management or related behavioral health experience.
• Experience coordinating with multidisciplinary teams to deliver care.
• Ability to advocate for clients and coordinate with providers and services.

Responsibilities

• Conduct comprehensive assessments to identify needs, strengths and goals.
• Develop, implement and monitor individualized care plans with clients and stakeholders.
• Coordinate with healthcare providers, social services and community resources.
• Advocate for clients to access resources and remove barriers to care.
• Maintain accurate client records and progress notes.
• Provide crisis intervention and client safety support as needed.
• Facilitate group sessions and educational workshops.
• Participate in multidisciplinary team meetings to adjust care plans.
• Stay informed about behavioral health policy changes and best practices.
• Engage in ongoing professional development to enhance skills.
