Job summary

Banner Health is seeking a Crisis and Intake Therapist to assess and coordinate behavioral health care across hospital and telehealth settings. The role supports patients and families experiencing serious mental health or substance use disorders and works with multidisciplinary teams in Banner facilities and partner sites.

The position requires a Master’s degree in counseling or related field, licensure, and BLS, with a schedule that may include days, nights, weekends, and holidays to meet

Qualifications

• Requires Master’s degree in counseling, behavioral health, or related field to obtain licensure.
• Requires one of several professional licenses (LMSW, LCSW, LPC, LAC, LASAC, LISAC, LCMFT or MFT).
• Requires BLS certification.
• Requires about 2 years of clinical experience.

Responsibilities

• Perform emergent/non-emergent behavioral health assessments and determine level of care.
• Coordinate inpatient/outpatient admissions and care transitions.
• Document accurately in the medical record and follow physician orders when applicable.
• Assist with discharge planning and collaboration with families and providers.
• Communicate effectively with multidisciplinary teams and external agencies.
