Job summary

Addus Home Care is seeking a Family Service Specialist to support families in crisis under DCFS programs. You will work in home environments, assist with court-ordered services, prepare progress reports, and connect clients with housing and community resources.

Qualifications

• Must be at least 21 years old.
• Associate degree or two years of social work experience required.
• Valid driver's license, dependable automobile, proof of auto insurance.
• Must pass CANTS and other background checks.

Responsibilities

• Work with families in crisis through DCFS and state contracts.
• Prepare written reports and maintain records and court testimony.
• Prepare and sign monthly client progress reports with goals and service dates.
• Report child abuse and neglect; notify supervisor of significant events.
• Provide home-based teaching on homemaking, parenting, and resources.
• Arrange supervised/unsupervised visitation and transport clients as needed.
• Help families locate housing and community resources.
