Job summary

Connections Health Solutions is seeking a Case Manager to facilitate admission intakes, treatment planning, and discharge coordination for individuals in crisis. You will collaborate with an interdisciplinary team to coordinate care for clients, their families, and community partners, ensuring thorough documentation in the EHR.

Required are a Bachelor’s degree in behavioral health or related field and QMHP credentials, plus knowledge of the Virginia behavioral health system.

Qualifications

• Bachelor's degree or higher in behavioral health or related field is required.
• QMHP credential or eligibility strongly preferred.
• Knowledge of Virginia's behavioral health system and related resources.

Responsibilities

• Facilitate admission intakes and discharge planning in crisis settings.
• Coordinate with interdisciplinary treatment teams to ensure aligned care.
• Develop individualized treatment plans and obtain necessary signatures.
• Gather collateral information to assess treatment and discharge needs.
• Maintain accurate EHR documentation of interventions and progress.
• Navigate Virginia involuntary treatment processes and DBHDS requirements.
