Job summary

Connections Health Solutions is seeking a Licensed Mental Health Professional to join our crisis care team in Virginia. You will provide psychoeducation, assessment, treatment planning, and discharge planning for individuals in crisis, coordinating with a multidisciplinary team to ensure safe, person-centered care.

Qualifications

• Master's or doctoral degree in Counseling, Social Work, Psychology, Marriage and Family Therapy, Psychiatric Nursing, or related behavioral health field
• Current Virginia licensure as LPC, LCSW, LMFT, Licensed Clinical Psychologist, LSATP, PMHNP, or other LMHP designation recognized in VA
• Must be able to perform Safe Clinch restraint techniques
• Company policy requires vaccination or approved exemption prior to start date
• State of Virginia Fingerprint Clearance required and background checks
• Must obtain Food Handlers Card for VA (BHS/RSS positions)

Responsibilities

• Educates and informs individuals about treatment consents and authorizations
• Participates regularly in interdisciplinary treatment team meetings
• Develops and implements an individualized person-centered Treatment Plan and reviews with each individual and involved parties
• Gather collateral to provide a thorough assessment of treatment needs and discharge planning
• Coordinate care and provide referrals to ensure services are coordinated with facility staff and external providers
• Identify gaps in service needs and recommend actions for discharge
• Collaborate with the treatment team to engage, monitor, and communicate with individuals while ensuring safety
• Maintain clinic records and document all services in medical records
• Maintain knowledge of VA involuntary treatment process and DBHDS/DMAS system
• Participate in supervision and ongoing training to maintain competency
• Complete required training, attend meetings, and maintain competencies
• Coordinate civil commitment process with stakeholders
• Provide daily programming/groups
• Perform all other duties as assigned
