Job summary

Connections Health Solutions is seeking a Case Manager in Chantilly, VA to facilitate admissions, develop treatment plans, and coordinate discharge planning for individuals in crisis. You will collaborate with an interdisciplinary team, coordinate with community partners, and maintain comprehensive case records.

Requires a Bachelor’s in behavioral health and QMHP certification, with knowledge of Virginia’s behavioral health system.

Qualifications

• Bachelor’s degree in behavioral health or related field.
• QMHP certification.
• Knowledge of Virginia behavioral health system and resources.

Responsibilities

• Facilitate admission intakes and treatment planning.
• Coordinate discharge planning and collateral collection.
• Assist in legal processes and implementing person-centered treatment plans.
• Maintain accurate EHR documentation of services and activities.
• Coordinate care with interdisciplinary team and community partners.
• Conduct daily programming/groups and ensure safety and compliance.
