Job summary

MIND 24-7 is redefining access to behavioral health care with 24/7 walk-in mental health services for all ages. The Crisis Case Manager will coordinate discharge planning, psychiatric medication management, and supportive services to ensure seamless transitions and timely follow-up.

Ideal candidates hold an Associate’s or Bachelor’s degree in healthcare or related field, 1–2 years in case management, and a Fingerprint Clearance Card. Bilingual English/Spanish is a plus.

Qualifications

• 1–2 years of experience in case management, care coordination, or behavioral health services.
• Fingerprint Clearance Card or ability to obtain upon hire.
• Bilingual (English/Spanish) is a plus.
• Strong communication, time management, and organizational skills.

Responsibilities

• Coordinate discharge planning and ongoing care for patients under psychiatric medication management.
• Schedule follow-up appointments with outpatient psychiatric providers and therapists.
• Support patient access to prescriptions, transportation, insurance coverage, and social services.
• Communicate with pharmacies, families, guardians, and care teams to ensure continuity of care.
• Document case management activities in the EHR system.
• Participate in interdisciplinary team meetings and contribute to holistic care planning.

Crisis Case Manager - Behavioral Health

Mind 24-7 is redefining how people access behavioral health care by offering 24/7 walk-in mental health services for children, adolescents, and adults. We provide immediate access to care—without waitlists or ER delays—and support individuals through crisis stabilization, medication management, and care transitions. Our Express Plus program offers short-term observation and psychiatric support for patients who are stable but require medication management and care coordination.

Your Role & Impact

• Coordinate discharge planning and ongoing care for patients under psychiatric medication management.
• Schedule follow-up appointments with outpatient psychiatric providers and therapists.

What We’re Looking For

• Associate’s or Bachelor’s degree in healthcare, behavioral health, human services, or related field preferred.
• 1–2 years of experience in case management, care coordination, or behavioral health services.
• Level 1 Fingerprint Clearance Card (or ability to obtain upon hire
• Strong communication, time management, and organizational skills.
• Familiarity with EHR systems and HIPAA compliance.
• Bilingual (English/Spanish) is a plus.
