Job summary

Lutheran Social Services in Milwaukee is seeking a Crisis Care Coordinator for the REACH Youth Crisis Team to provide trauma-informed, person-centered care coordination for youth and families. This role focuses on preventing out-of-home placements and linking clients to vital community resources.

Responsibilities include assessments, care coordination, and collaboration with families and providers. Must be flexible for after-hours on-call shifts and hold a bachelor's degree in human services

Qualifications

• Bachelor's degree in human services (e.g., Social Work, Psychology, Counseling) required.
• Minimum 1 year of mental health service experience.
• Valid driver's license and reliable transportation.

Crisis Care Coordinator - REACH Youth Crisis Team (Milwaukee, WI)

The position is full time, benefits eligible opportunity. Hours are Monday through Friday, but will require flexibility to include evenings to meet the needs of clients. Participation in on-call rotation is required.
