Job summary

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a Care Coordinator for the CLTS Waiver Program in Walworth County, WI. This full-time role works M-F with a hybrid work option and serves as the primary contact for families navigating services.

You'll assess children's needs, develop service plans with families and providers, coordinate teams, and maintain documentation to meet regulatory timelines. Travel is required in the community.

Qualifications

  • Bachelor's degree in Social Work or related human services field is required.
  • Minimum 1 year of experience working with children/youth with disabilities is preferred.
  • Bilingual fluency in Spanish is preferred but not required.
  • Proficient in computer systems and electronic health records (EHR).
  • Strong interpersonal and organizational skills are essential.
  • Valid driver’s license and reliable transportation (MVR check required).

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our CLTS Waiver Program team in Walworth County!

The Children’s Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services with compassion and care.
