Job summary

Memorial Regional Health in Craig, CO seeks a Crisis Behavioral Health Peer Advocate to provide peer-based support, crisis response, and care coordination for patients and families within the hospital system.

You will connect patients with community resources, assist in short-term case management, and collaborate with MRH teams to promote trauma-informed, person-centered care across services.

Qualifications

• Must be 18+ years old and legally authorized to work in the United States.
• Must pass a background check.
• Must pass a pre-employment drug screen and breath alcohol test (if applicable).
• Must complete an Employee Health meeting prior to starting employment.
• Two years of experience in behavioral health evaluations, advocacy, or substance use fields.

Responsibilities

• Engages with patients and stakeholders using trauma-informed and culturally sensitive communication.
• Maintains client privacy and confidentiality within the system of advocacy and HIPAA compliance.
• Coordinates behavioral health projects, meetings, trainings, and schedules.
• Attends and documents meetings with stakeholders and provides insights and expertise.
• Assists in departmental administration, scheduling, and operations as requested.
• Performs SDOH assessments and referrals per regulations.
• Assists patients in crafting safety plans to reduce suicide risk as appropriate.
• Conducts high-quality evaluations of suicide risk and social needs for medical staff coordination.
• Refer patients to state-approved facilities and coordinate admissions/transfers.
• Recognizes behavioral health concerns and makes recommendations or referrals.
• Assists with behavioral health discharge planning and continuity of care.
• Acts as patient advocate and resource person to staff and patients.
• Maintains updated resource information for MRH systems.
• Develops and maintains relationships with community partners.
• Provides training on behavioral health topics for MRH employees and external stakeholders.
