AdventHealth is seeking a highly qualified professional to serve as the primary liaison between clients, families, managed care representatives, and behavioral health staff. This role ensures continuity of patient care after assessments and supports implementing referral recommendations.
The position requires a Master’s degree and relevant clinical licenses (CNS, LCSW, LMFT, LPC, PSYCH). You will work in a faith-based health system committed to holistic care across a broad network.

Serves as primary communication liaison between clients, family members, managed care representatives, and behavioral healthcare personnel. Assures the continuity of patient care upon completion of the assessment by assisting in the implementation of the referral recommendation. Provides therapeutic and educational services to patients, family, staff, and community. Appropriately assesses suicide, violence, and elopement potential. Facilitates appropriate milieu management within the emergency departments and medical units.
Pay Range: $29.82 - $55.47
