JOB DESCRIPTION:

SUMMARY OF POSITION

The Criminal Justice Case Manager (CJCM) is part of a multidisciplinary clinical team who serves as the bridge between behavioral health services and the criminal justice system. The CJCM will work to provide intensive services to released inmates or those at imminent risk of incarceration who suffer from mental health illness and/or substance abuse disorder. The CJCM will ensure clients make connections to behavioral, medical health and/or substance abuse treatment. CJCM will facilitate direct referrals for housing, employment, legal, educational, peer-support, entitlements, and all other essential community-based needs.


This is a community-based position with local travel expected within Montgomery County and surrounding counties as needed. Mileage is reimbursed.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provide coordination of care services which assist clients in identifying, accessing, and coordinating resources that are supportive in achieving the client’s successful integration into the community or away from the criminal justice system.
  • Offer Mental Health Crisis Management Training to sheriff personnel, alternative transporting agencies, other criminal justice personnel in the judicial districts, and mental health center staff.
  • Provide basic mental health training for the Tennessee Correctional Institute’s Basic training as scheduled by TCI in the judicial district.
  • Submit monthly statistical reports in a timely manner.
  • Attend and participate in TDMHDD quarterly network meetings.
  • Conduct thorough assessments of needs using state mandated assessment tools.
  • Prepare and maintain confidential case records and documentation in the electronic health record and TN-Wits documentation program to ensure confidentiality and control access to sensitive information.
  • Consult with and attend meetings with service providers and community partners on resource-related issues.
  • Collaborate with clinical team to perform risk management and crisis intervention services as necessary.
  • Develop network of trusted contacts of organizations and agencies in which to refer and coordinate care.
  • Participate in multidisciplinary team meetings.

KNOWLEDGE, SKILLS & ABILITIES

  • Knowledge of and experience working with local social service, public health, and criminal justice system
  • Experience working with high risk/high need population; preference for crisis and legal experience.
  • Must demonstrate leadership capacity, ability to work within a team, but also be able to work independently.
  • Excellent written and oral communication skills.
  • Must be highly organized, detail oriented, and reliable.
  • Demonstrated ability to remain calm in the midst of difficult circumstances and emergencies. Flexible and able to adapt to change. Ability to respond in a professional manner in all situations.
  • Ability to work with all clients regardless of past criminal charges or convictions.
  • Ability to communicate clearly and effectively via oral or written means.
  • Ability to present a friendly and positive demeanor to all clients, their families, staff, court/probation/jail personnel, treatment providers, and the general public.
  • Ability to make oral presentations and use PowerPoint to train law enforcement.
  • Working knowledge of Outlook, Word, Excel, and PowerPoint. History of documenting within an electronic health record preferred.
  • Ability to think analytically and evaluate the impact of case management recommendations.
  • Ability to set work priorities and to evaluate and create solutions to work related problems.
  • Strong math skills. Ability to assist clients in developing a budget.
  • Crisis management skills preferred.
  • Knowledge of case management services and local community resources.

QUALIFICATIONS

Education Level

  • Bachelor’s Degree in Social Work, Sociology, Psychology or related field required
  • Master’s Degree preferred

Years of Experience

  • One or more year’s case management experience required
  • Must have one or more years' experience in a criminal justice affiliated environment
