Job summary

Family Pride of Northeast Ohio, Inc. seeks a compassionate Case Manager (TBS Staff) to support justice-involved individuals through the Chardon Municipal Court and community providers, delivering services in homes, virtual, and in the field.

You will conduct needs assessments, coordinate services, monitor progress, and communicate with judges, probation officers, and partners to promote stability and compliance with court conditions.

Qualifications

  • Bachelor’s degree or higher in a related field.
  • Experience with justice-involved populations, behavioral health, or case management.
  • Strong communication and collaboration with multidisciplinary teams.
  • Ability to build rapport with high-risk, vulnerable populations.
  • Knowledge of community resources and counseling/social work practices.

Responsibilities

  • Conduct comprehensive needs assessments across legal involvement, MH/SUD, employment, housing, transportation, and basics.
  • Develop individualized action plans with participants.
  • Guide participants in understanding bond conditions, probation requirements, and court expectations.
  • Link participants to behavioral health, substance use treatment, healthcare, and community resources.
  • Coordinate Medicaid/insurance transportation to court hearings and appointments.
  • Maintain timely records in the agency’s EHR system.
  • Collaborate with judges, probation officers, and service providers.

As TBS Staff with Family Pride, you have the opportunity to make a meaningful difference in the lives of individuals and families throughout Northeast Ohio. Our TBS provide services where clients are most comfortable and where support is needed most—including homes, virtual, communities, and office settings.

Position Overview

Family Pride of Northeast Ohio, Inc. is seeking a compassionate and dedicated Criminal Justice TBS to serve as a liaison between the Chardon Municipal Court, justice-involved individuals, and community-based service providers. This role focuses on assessing needs, removing barriers to success, and coordinating care to improve compliance with court requirements and promote long-term stability of justice-involved individuals. The coordinator provides structured support, connects individuals to essential services, and ensures timely communication with court personnel.

What You’ll Do
  • Conduct comprehensive needs assessments to identify needs related to legal involvement, mental health and substance use disorders (MH/SUD), employment, housing, transportation, and basic needs
  • Assist individuals in prioritizing needs and developing individualized action plans
  • Provide structured support to participants in understanding and complying with bond conditions, probation requirements, and court expectations
  • Link individuals to behavioral health, substance use treatment, healthcare services, and other community resources to reduce barriers and promote stability
  • Assist with insurance enrollment, reactivation, and navigation, as needed
  • Coordinate Medicaid and insurance-funded, community-based transportation to ensure attendance at court hearings, probation appointments, and treatment services
  • Maintain consistent, professional communication with judges, probation officers, court staff, and service providers
  • Document all services and maintain accurate, timely records within the agency’s electronic health record (EHR) system
  • Share required updates and reports with Chief Probation Officers and designated court personnel
  • Monitor participant engagement, service utilization, and compliance with court‑ordered conditions
  • Utilize trauma‑informed, strengths‑based, and culturally responsive approaches to support engagement, accountability, and progress
  • Identify and respond to crisis situations, coordinating emergency or clinical services as needed and reporting safety concerns according to protocol
  • Participate in case staffing meetings, court sessions, and interdisciplinary team collaboration as requested
  • Track and report key performance metrics, including individuals served, Medicaid enrollment outcomes, referrals completed, transportation coordination, and compliance support activities
  • Collaborate with community partners, employers, natural supports, and family members to advocate for and support participant success
  • Manage a balanced caseload with the support of supervision
  • Perform other duties as assigned

  • Recognize symptoms of mental illness and substance abuse
  • Be Knowledgeable of community resources and counseling/social work practices with high‑risk populations
  • Work effectively with people of diverse educational and cultural backgrounds
  • Be proficient in computer use, including common software applications, web‑based platforms, and professional email communication
  • Possess excellent verbal, written, and interpersonal communication skills
Work Environment & Travel
  • Work Environment: This position operates in the Chardon Municipal Court, a professional office environment, and throughout the community. It routinely uses standard office equipment such as computers, phones, photocopiers, and filing systems.
  • Physical Demands: The role requires the ability to sit or stand for extended periods, use hands to operate office equipment, and communicate effectively in person and via electronic means. Occasional lifting of materials up to 25 pounds may be required.
  • Sensory Requirements: Must be able to communicate clearly and effectively, both verbally and in writing. Requires visual acuity to read and interpret documents, data, and computer screens.
  • Schedule and Hours: This is a full‑time, exempt position. The position is expected to work a standard workweek, with occasional evening or weekend hours to attend meetings, events, or to respond to urgent organizational needs.
  • Travel: This position requires regular local travel to provide services to justice‑involved individuals in a variety of settings, including the court, community locations, partner agencies, and, when appropriate, clients’ homes. Travel may also be required to attend meetings with court personnel, service providers, and community partners. Occasional regional travel may be necessary for training or professional development.
  • The employee must possess and maintain a valid Ohio driver’s license, meet insurability requirements in accordance with the organization’s vehicle use policy, maintain appropriate auto insurance coverage, and have reliable access to a vehicle.
