Job summary

Cascadia Health is seeking a Counselor III for Woodland Park Health Center in NE Portland. The role focuses on timely behavioral health assessments, service planning, and individual/group interventions within Integrated Team-Based Care.

You will coordinate care with a multidisciplinary team, promote trauma-informed, person-first approaches, and engage with clients, families, and community resources to support recovery and safety.

Qualifications

• Master's degree in behavioral science from an accredited college or university.
• Credential eligibility as QMHP per Oregon Administrative Rules (OAR).
• QMHP credentialing maintained per MHACBO requirements.

Responsibilities

• Complete timely behavioral health assessments and develop service plans.
• Provide individual and group counseling per approved plans.
• Coordinate with interdisciplinary team for case planning and reviews.
• Monitor and revise service plans based on client progress and needs.
• Engage clients and families to support person-centered goals.
