Job summary
La Casa Via Transitional Care Center is seeking a Certified Occupational Therapist Assistant (COTA) in Walnut Creek, CA. This full-time position offers an hourly pay range of $40.00–$45.00, commensurate with experience, within a skilled nursing rehabilitation setting.

Qualifications
- Licensed COTA in California.
- Graduate of an accredited Occupational Therapy Assistant program.
- Successful NBCOT examination completion.
- Current CPR/BLS certification.
Responsibilities
- Implement OT treatment plans under the supervising OT.
- Deliver skilled interventions for ADLs, IADLs, and upper-extremity functions.
- Monitor patient progress and communicate with the OT about concerns.
- Assist in data collection for progress and plan-of-care updates.
- Instruct patients and caregivers in home programs and safe equipment use.
- Support discharge planning and continuity of care.
