Job summary

City Scape Rehabilitation and Care Center in Denver, Colorado is seeking a Certified Occupational Therapist Assistant. This in-house therapy team addresses patient needs and collaborates with the IDT to provide rehabilitative services to residents in a close-knit setting.

The role reports to the Director of Rehabilitation Services. Active state licensure is required, with adherence to NBCOT/AOTA and state practice acts.

Qualifications

• Must hold active state licensure as a Certified Occupational Therapist Assistant.
• Adherence to NBCOT/AOTA standards and state practice acts.

Responsibilities

• Provide rehabilitative therapy to patients with cognitive and/or ADL physical functioning disorders following regulatory and clinical practice requirements.
• Document objective measures and justification for continued or discontinued services.
• Utilize assessments and evidence-based practice to support interventions.
• Administer procedures as part of the rehabilitation plan (ADL retraining, therapeutic activities, exercises, modalities, devices).
• Monitor patient progress and adjust treatments in medical records.
• Communicate progress with supervisor and health team; provide discharge summaries as required.
• Maintain documentation in electronic health records and ensure accurate treatment minutes.
• Participate in meetings and required trainings.
