Job summary

Continuum Therapy Partners, LLC is seeking a Certified Occupational Therapy Assistant for a PRN daytime schedule at Claymont Health & Rehabilitation in Uhrichsville, Ohio. You will provide high-quality therapy under supervision, helping maintain continuity of care with the same standards as the full-time team.

The role supports individualized treatment plans, documents patient progress, and educates patients on adaptive equipment and home exercise programs.

Qualifications

• Associate Degree in Occupational Therapy Assistant Program required
• Current, unencumbered state COTA License; NBCOT Certification required
• 1 year of previous experience as a Certified Occupational Therapy Assistant preferred

Responsibilities

• Implements individualized treatment plans under the supervision of an Occupational Therapist
• Documents patient progress and reports changes in patient status to the supervising OT
• Educates patients on adaptive equipment and home exercise programs
