Job summary

Green Hills Center for Rehabilitation and Healing is seeking a Certified Occupational Therapy Assistant (COTA) to provide patient-centered care under the supervision of an Occupational Therapist. You will implement treatment plans, document progress, and communicate with the OT team to ensure goals are met.

The role emphasizes collaboration with a multidisciplinary team, timely documentation, professionalism, and delivering high-quality geriatric care in a facility setting.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist
• Experience with long-term care and/or geriatric care preferred
• Well versed with documentation writing
• Document patient's progress on weekly progress note in a timely fashion
• Collaborates with all disciplines to provide best patient centered treatment and care
• Exceptional patient service skills and professionalism
• Report to work on time, adhere to scheduled hours and demonstrate professionalism
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide feedback to supervision occupational therapist via documentation or facility policy requirements.
