Green Hills Center for Rehabilitation and Healing is seeking a Certified Occupational Therapy Assistant (COTA) to provide patient-centered therapy under supervision of an Occupational Therapist. You will implement treatment plans, document progress, and collaborate with the rehab team to support long-term care residents.

Qualifications include a valid state license, experience in geriatric care preferred, and strong documentation.

Qualifications

• Must hold a valid state license/registration.
• Provide comprehensive treatment under supervision of an OT.
• Experience in long-term or geriatric care preferred.
• Strong documentation skills.
• Timely weekly progress notes.
• Collaborates with all disciplines.

• Duties assigned by Director of Rehabilitation.

Responsibilities
