Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to perform treatments under the supervision of an Occupational Therapist. You will follow set goals, document patient progress weekly, and collaborate with the rehabilitation team to ensure patient-centered care.

The role requires a valid license, experience in long-term or geriatric care preferred, and punctual professional conduct in a facility setting.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Excellent documentation and progress note writing.
• Collaborates with disciplines to provide patient-centered care.

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment per goals developed by the OT.
• Document patient progress in weekly progress notes.
• Report to Director of Rehabilitation and adhere to facility policy.
