Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to provide treatment under OT supervision in a subacute/geriatric setting. You will document patient progress, contribute to care plans, and work with a multidisciplinary team to ensure high-quality patient-centered care.

Applicants must hold current state license/registration as a COTA and have strong documentation skills.

Qualifications

• Must hold a valid professional license and registration for state of practice (New Jersey).
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills and timely weekly progress notes.

Responsibilities

• Deliver treatment under supervision of an Occupational Therapist.
• Document patient progress and communicate updates per facility policy.
• Collaborate with disciplines to provide patient-centered care.
• Ensure timely completion of duties and maintain professionalism.
