Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to deliver evidence-based treatments under the supervision of an Occupational Therapist. You will follow OT-directed goals and document progress to support patient-centered care in a subacute/geriatric setting.

Responsibilities include timely progress notes, collaboration with interdisciplinary teams, and adhering to facility policies while delivering high-quality patient care.

Qualifikationen

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Document patient progress with weekly progress notes in a timely fashion.
• Collaborates with all disciplines to provide best patient-centered treatment and care.

• Carry out best practice treatments under supervision of the Occupational Therapist.
• Provide treatment by adhering to goals developed by the OT.
• Provide feedback to the supervising occupational therapist via documentation or facility policy.
