Job summary

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide skilled rehab services to adult and geriatric patients in a skilled nursing and post-acute setting at Stoney Point Meadows in Cedar Rapids, IA. You will work under the supervision of a licensed OT to implement treatment plans that improve ADLs, mobility, strength, cognition, and independence.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Occupational Therapy Assistant license (or eligibility) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in SNF and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising OT.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
