Chatham Hills Subacute Care Center in New Jersey seeks a Certified Occupational Therapy Assistant (COTA) to deliver therapy under the supervision of an Occupational Therapist. You will implement treatment plans, document patient progress weekly, and collaborate with the rehab team to ensure patient-centered care.

Ideal candidates hold an active state license/registration, have strong documentation skills, and demonstrate punctuality and professionalism in a long-term care setting.

Qualifications

• Must hold a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist.
• Experience in long-term/geriatric care preferred.
• Strong documentation writing skills.
• Document patient progress on weekly notes and maintain timely reporting.
• Collaborate with all disciplines to provide patient-centered care.

Responsibilities

• Carry out best-practice treatments under OT supervision.
• Provide feedback to supervising OT via documentation as required.
• Collaborate with rehab team to deliver quality care.
• Maintain professionalism and punctuality in all shifts.
• Perform other duties as assigned by Director of Rehabilitation.
