Job summary

Chatham Hills Subacute Care Center in Fanwood, NJ is seeking a Certified Occupational Therapy Assistant to carry out best practice treatments under the supervision of the Occupational Therapist. You will provide care aligned with goals set by the OT and document progress.

Candidates should hold a valid state license, prefer experience in long-term or geriatric care, and demonstrate strong documentation, teamwork, and punctuality.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Document patient progress on weekly progress note.
• Collaborates with all disciplines for patient-centered care.
• Punctual and professional; adhere to scheduled hours.
• Perform duties as assigned by Director of Rehabilitation.

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment aligned with goals developed by the OT.
• Provide feedback to supervising OT via documentation.
• Collaborates with all disciplines for patient-centered care.
• Maintain punctuality and professionalism.
• Perform all other duties assigned by Director of Rehabilitation.
