Job summary

Community Skilled Health Care Centre in Warren, OH is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to provide skilled OT services under the supervision of a licensed Occupational Therapist, helping residents regain independence and improve quality of life.

Qualifications

• Current COTA license/certification.
• Graduate of an accredited Occupational Therapy Assistant program.
• Experience in skilled nursing, long-term care, or rehabilitation preferred.
• Strong communication and organizational skills.

Responsibilities

• Provide occupational therapy treatments according to established care plans and physician orders.
• Assist residents in improving functional abilities, mobility, independence, and quality of life.
• Implement therapeutic activities designed to improve daily living skills.
• Monitor resident progress and communicate changes with the Occupational Therapist and interdisciplinary team.
• Document treatments, resident responses, and progress accurately and timely.
• Collaborate with nursing, therapy, and other departments to support resident‑centered care.
• Maintain compliance with facility policies, safety standards, and regulatory requirements.
