Job summary

Lakeport Post Acute in Lakeport, California is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to strengthen our growing rehabilitation team in a skilled nursing and post-acute setting.

You will support the Occupational Therapist by implementing individualized treatment plans, delivering ADL and mobility interventions, documenting progress, and educating residents and families. New graduates are welcome; sign-on bonus and relocation assistance are available.

Qualifications

Active California COTA license.
Graduate of an accredited Occupational Therapy Assistant program.
Strong communication and teamwork skills.
New graduates are encouraged to apply.

Responsibilities

Assist the OT in implementing individualized treatment plans.
Provide interventions to improve ADLs, mobility, and functional independence.
Monitor patient progress and communicate updates to the supervising OT.
Educate residents, families, and caregivers on therapy techniques and safety practices.
Maintain accurate and timely documentation per regulatory standards.
Collaborate with physicians, nursing staff, and interdisciplinary teams.
Support patient-centered goals to maximize recovery.
