Elara Caring is hiring a Certified Occupational Therapist Assistant to join our home-based care team in Kokomo, Indiana. The role focuses on delivering patient-centered occupational therapy per a physician-approved plan, visiting patients regularly, monitoring progress, and collaborating with the care team to adjust treatment as needed.

This position requires graduation from an accredited OTA program, current COTA certification in Indiana, and at least one year of clinical experience.

Formación

• Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association.
• Current, unrestricted COTA certification in the state of work.
• Minimum one year of experience as a COTA in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.

Responsabilidades

• Provide occupational therapy services by following the care plan developed by the Occupational Therapist.
• Visit patients regularly and monitor their progress during each visit.
• Report changes in a patient's condition to the Occupational Therapist, physician, and care team.
• Follow physician orders and the established plan of care.
• Teach patients and caregivers home therapy exercises and safe techniques.
• Work closely with the care team to provide coordinated, high-quality patient care.
• Complete accurate and timely documentation for each patient visit.
• Help reduce avoidable hospital and emergency room visits through proactive patient care.
• Stay up to date on healthcare regulations and agency policies.
• Work in a physically active role that may include lifting, transferring patients, and occasional irregular or on-call hours.
