Elara Caring in Indiana is hiring a Certified Occupational Therapist Assistant to join our dedicated home care team. You will deliver therapy services under the plan of care, monitor patient progress, and educate patients and caregivers on exercises to support independence.

Required are an accredited COTA program graduate, active COTA license, one year of clinical experience, and ability to lift up to 50 pounds.

Qualifikationen

• Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association.
• Current, unrestricted COTA certification in the state of work.
• Minimum one year of experience as a COTA in a clinical care setting.
• Ability to frequently lift, push, pull, and transfer up to 50 pounds; reliable transportation.

Aufgaben

• Provide occupational therapy services per the plan of care.
• Visit patients regularly and monitor progress.
• Report changes to the Occupational Therapist and care team.
• Follow physician orders and care plan.
• Teach home therapy exercises to patients and caregivers.
• Collaborate with care team for high-quality patient care.
• Complete accurate and timely documentation for visits.
• Help reduce avoidable hospital and ER visits through proactive care.
• Stay up to date on healthcare regulations and agency policies.
• Perform physically active duties including lifts and transfers as needed.
