Job summary

North Ridge Health and Rehab is seeking a Certified Occupational Therapy Assistant (COTA) to join our Therapy Team. Plan, organize, develop, and direct Occupational Therapy Services in accordance with applicable standards to ensure high-quality resident care.

As a Per Diem/PRN role, you will screen residents, develop treatment plans, communicate results to families and the interdisciplinary team, and maintain accurate documentation and billing.

Qualifications

  • Associates Degree in Occupational Therapy.
  • WI OTA licensure.
  • Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

  • Conduct regular screening of residents to determine best intervention for each patient.
  • Perform treatment plans; follow short and long term goals per plan of care.
  • Interpret and communicate evaluation findings to residents, families, and team members.
  • Establish individual resident therapy schedules with team members.
  • Conduct therapy according to treatment plan and ensure proper documentation and billing.
  • Perform all other duties as assigned to treat patients.
  • Comply with CMS documentation standards daily.

North Ridge Health and Rehab is seeking for a Certified Occupational Therapy Assistant (COTA) to join our Therapy Team. As a COTA you will be responsible to plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Status: Per Diem/PRN

Responsibilities
  • Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice

  • Perform treatment plan to residents; follow short and long term goals and methods to achieve goals based off of the established plan of care by supervising therapist

  • Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

  • Establish individual resident therapy schedules in cooperation with other team members

  • Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

  • Perform all other duties as assigned and as required to effectively treat patients

  • Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications
  • Associates Degree in Occupational Therapy

  • WI OTA licensure

  • Ongoing continuing education and professional development to maintain current licensure and certification
