Job summary

CHP Health is seeking a compassionate Licensed Practical Nurse (LPN) to join our healthcare team as a Discharge Planner in Placer County Jail. This full-time position (30 hours per week) provides high-quality nursing care in a correctional setting under supervision of RNs and physicians.

Qualifications

• Graduate of an accredited practical/nursing program
• Current, unrestricted LPN license in the state of employment and CPR card
• Minimum 1 year clinical experience; correctional healthcare experience preferred

Responsibilities

• Follow nursing processes and standards per policies, procedures, and accreditation requirements
• Coordinate care with HSA, DON, and RN staff within the facility
• Utilize EHR with minimal supervision; assist with staff training as requested
• Communicate effectively with clients, peers, and employees; maintain clear lines of communication during issue-resolution
• Document patient history and vital signs; assist with examinations and treatments
• Administer medications and document on the MAR in the EHR
• Perform venipuncture and IV therapy as approved by supervisor
• Assist with daily triage/sick call, minor procedures, and transcribe orders from the Provider
• Initiate prescribed treatment plans and ensure compliance with policies
• Support quality improvement efforts and patient safety initiatives
