Job summary
Wellpath is hiring a Nurse Discharge Planner LVN to develop and coordinate clinically informed reentry care plans for CalAIM-eligible individuals. You will perform basic nursing assessments, review health records, coordinate referrals, and facilitate warm handoffs to managed care plans and community providers.
The role requires active nursing licensure in the state, CPR certification, and the ability to obtain security clearance for the facility.
Qualifications
- Graduation from an accredited School of Nursing is required.
- Bachelor's degree in Social Work, Business Administration, or other health-related fields preferred.
Responsibilities
- Provide discharge planning services for medical, mental health, MAT, and related programs.
- Identify release-planning needs and coordinate medications and medical needs upon release.
- Arrange referrals and appointments with community providers.
- Facilitate psychoeducational sessions, benefit applications, and resumption of services.
- Maintain relationships with facility staff and partners while adhering to security regulations.
- Document services in the health record and ensure compliance with DHCS CalAIM requirements.
