Job summary

State of Colorado Department of Corrections seeks a Corrections Case Manager II to lead CM I staff and coordinate offender re-entry planning. This role focuses on continuity of care from intake to release and requires collaboration with internal and external partners.

Minimum qualifications include four years of corrections experience with at least one year in case management, strong supervisory and communication skills, and a commitment to evidence-based practices.

Qualifications

• Four (4) years of corrections experience with at least one year providing case management services to incarcerated individuals.
• Experience must be described in your application with classification, dates and duties.
• Corrections-related experience includes direct service to detained individuals or supervising offenders in a program.
• Education in Criminal Justice, Social Work, Psychology, or related field plus four years of experience may satisfy MQs.

Responsibilities

• Oversee work product of CM I staff, provide training and coaching.
• Guide offenders from intake to release with CTAP objectives and parole planning.
• Coordinate with internal/external stakeholders to ensure continuity of care.
• Utilize motivational interviewing and cognitive-based interventions in case planning.
• Track service delivery, assess custody levels and programming needs.
• Coordinate earned time grants and community corrections referrals.
• Serve as primary contact for facility administration, Parole Board, and partners.
