Job summary

Dreams 2 Reality Recruitment is seeking a qualified Nurse Practitioner to deliver comprehensive primary care within a structured correctional healthcare setting. The role involves evaluating acute and chronic conditions, managing ongoing patient care, and collaborating with physicians and facility staff.

The ideal candidate has a Master’s degree in Nursing, DEA registration, and current licensure as FNP/AGNP/NP, with at least one year of primary care and care management experience.

Qualifications

• Must hold a Master’s degree or higher in Nursing.
• Minimum 1 year of primary care experience.
• Minimum 1 year of care management experience.
• DEA registration and active licensure as FNP/AGNP/NP.
• Current ARC BLS or AHA BLS and AHA ACLS certifications.

Responsibilities

• Perform comprehensive patient assessments and examinations.
• Diagnose and manage acute and chronic conditions.
• Provide sick-call coverage and same-day evaluations.
• Develop and monitor individualized treatment plans.
• Coordinate with physicians, nurses, and facility staff.
• Educate patients on medications, disease management, and preventive care.
• Maintain accurate EMR documentation.
• Recognize changes in patient condition and refer as needed.
• Adhere to clinical, regulatory, safety, and correctional healthcare standards.
• Promote patient safety and continuity of care.
