Qualifications

• License to practice nursing as a Licensed Practical Nurse issued by Ohio Board of Nursing per Ohio Revised Code 4723.
• LN: LPN dialysis certification may be required per policy.

Responsibilities

• Provides patient care as directed.
• Documents patient-related care timely in the electronic health record.
• Maintains patient medical records.
• Administers medications (oral, intramuscular; IV therapy if certified).
• Performs treatments and assists with doctor visits and sick call.
• Assists with chronic care clinic management and multidisciplinary team activities.
• Responds to emergencies and provides emergency care and data collection.
• Collects vital signs and other samples; prepares patients for discharge/transfer.
• Performs inventory and orders medical supplies; supports healthcare staff.
