Job summary

Sentara RMH Medical Center in Harrisonburg, VA is seeking an RN, Integrated Care Manager to support Pulmonary Specialists and coordinate care across settings. This is a full-time, onsite role with daytime first shift.

The position emphasizes comprehensive care planning, utilization monitoring, and collaboration with the care team and patient/caregiver to manage transitions. Onsite in the Medical Office Building.

Qualifications

• 3 years of RN experience required.
• Pulmonary/complex outpatient care preferred.
• Active VA RN license or compact license required.
• BLS within 90 days of hire; specialty certification within 1 year for Integrated Care Management.

Responsibilities

• Coordinate comprehensive care across the continuum.
• Promote effective utilization and monitoring of health services.
• Collaborate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan based on assessment and evaluation.
• Work across Acute Care, Service Lines, Ambulatory/Community-based, Home Health and Long-Term Care settings.
