Family Service League Inc. in Bay Shore, NY is seeking a part-time Long Term Care Coordinator for our Ombudsman program.
The role focuses on empowering, educating, and advocating for residents in licensed facilities across Suffolk County. You will mentor volunteers, visit facilities lacking Ombudsman coverage, and ensure compliance with state regulations while maintaining clear reporting and documentation by deadlines.

SCHEDULE:
21 hours per week- 7 hours a day x three days a week.
SUMMARY:
Family Service League is seeking a part-time Long Term Care Coordinator for our Ombudsman program in Bay Shore, NY. The Coordinator will empower, educate, and advocate for residents in licensed long term care facilities in Suffolk County. Responsibilities include mentoring volunteers, visiting uncovered nursing homes, assisted living , adult homes and family type homes.

QUALIFICATIONS:
Bachelor’s Degree in human services, social work or related field of study required.
NYS Certified Ombudsman certification required - training provided and must be completed within 6 months.
A minimum of 3 years’ experience in working with older adult and disabled individuals preferred.
Strong interpersonal skills and the ability to deal effectively with a diverse range of people.
Understanding of aging, health care and long-term care issues is preferred.
Excellent organizations skills, ability to manage multiple tasks and meet deadlines.
Self-starter, ability to work independently, as a team and be self-motivated.
Excellent verbal and written communication skills, as well as presentation skills.
Computer proficiency, including Microsoft Office, required.
Valid, clean New York State Driver’s License required.
